Requirements

Requirements / Documents Needed:

Valid HHA certificate.
Physical (cannot be older than 3 months).
Drug screen test (cannot be older than 3 months).
If the PPD is positive then a chest x-ray is required.
Rubella & Rubeola blood work (from the lab).
Two (2) typed references.
Social security card
